See the records your patients share with you

Max Health members keep their health records in one place and decide who sees them. Verify your NPI and medical license once, and patients can share their records with you by name.

Sign in to apply

You apply with your Max Health account. Signing in creates one if you do not have one yet.

What you get

Verified once

Your individual NPI and your state medical license, checked against NPPES and the state board. One verification follows you wherever you practice.

Shared by the patient

Patients choose who sees their record, for how long and which parts. A verified practitioner is someone they can name.

No patient record needed

A practitioner account is all you need. If you ever want your own health record too, the same account holds both.

Independent or employed

Freelance and locum physicians verify on their own. Your license is yours, so it stays verified when you change where you work.

How it works

  1. Sign in with your Max Health account, or create one. You do not need a health record of your own.
  2. Apply with your Type 1 NPI and the state and number of your medical license. NPPES must list the NPI as active.
  3. We confirm the license with the state board and screen the HHS-OIG exclusion list. Your dashboard shows where your application stands.
  4. Once verified, patients can share their records with you by name, and clinician apps sign you in as a practitioner.

Common questions

Who can apply?

Clinicians licensed in a US state, DC or a US territory, with an active individual (Type 1) NPI. An organization NPI belongs to a practice, not to a person, so it cannot be used here.

What do you check?

That NPPES lists your NPI as active under your name, that the state board lists your license as active, and that you are not on the HHS-OIG List of Excluded Individuals and Entities.

Can I see any patient once I am verified?

No. Verification says who you are. A patient decides whether you see their record, which parts and for how long, and can withdraw that at any time.

What happens if my license lapses?

Once we learn of it, we withdraw the verification, and with it your access to the records patients shared with you. You can apply again when the license is active.

I work at a clinic. Do I apply here?

Yes. A license belongs to the clinician, so every clinician verifies individually, whether they work alone or for a practice.
